GeriMedRisk

GeriMedRisk® is an online or telephone service connecting clinicians (e.g. family doctors, nurse practitioners) with a collaborative team of geriatric specialists to support deprescribing and medication optimization in primary care and long-term care.

Program Description

Clinicians can use telemedicine (Ontario Telemedicine Network, Champlain BASE™ eConsult) to connect with GeriMedRisk specialists who work together to deliver a combined result, without introducing the patient to another prescriber. eConsult response times are within 7 business days. For urgent cases, telephone is available.

By giving physicians in both urban and remote areas access to geriatric experts, practitioners can obtain tailored geriatric drug information resources in order to optimize their older adult patients’ medication. GeriMedRisk uses health care resources more efficiently, and clinicians get the answers they need, when they need them, to better serve older adults.

The program also offers a free monthly webcast, Geriatric Clinical Pharmacology Rounds, which covers topics such as: “Drug Interactions: Beyond start low, go slow” and “Shake, Rattle, and Roll: The toxic effects of levodopa/carbidopa in the older adult.”

GeriMedRisk is a non-profit service supported by partnerships, sponsors, and grant initiatives. Partners include McMaster University, Canadian Mental Health Association Waterloo Wellington, St. Joseph’s Health System, the Ontario Centres for Learning, Research & Innovation in Long-Term Care, Grand River Hospital, Regional Geriatric Program Central, Ontario Poison Centre, and Ontario Telemedicine Network Hub.
