The Waterloo Wellington Local Health Integration Network (WWLHIN) is interested in developing and implementing an integrated clinical services plan for frail seniors. To assist with this objective, a consultation process was conducted with interested key stakeholders across disciplines and health care sectors, to identify current system strengths and ascertain gaps and barriers to address in order to develop a truly integrated system of care to meet the needs and optimize outcomes of frail seniors. This report describes the methods and results of this consultation, and discusses how these results could be used to inform the development of an integrated system of care for frail seniors in the WWLHIN.
